Production Toolkit: Contact Details and Roles

At Thursday's production meeting I asked everybody to confirm their contact details and here is the updated Contact Details Sheet which shows contact details and agreed roles:

I have put the lead person for each role in bold, and under that have listed the other crew members that will be doubling up and helping out in that role, as agreed at the meeting.

I have also created a notes field to hold important information.  This document has been distributed to the each member of the team via the team Facebook site.
